WHITE ROCK / SOUTH SURREY

In the community of White Rock-South Surrey, the Division of Family Practice has helped find regular primary care for more than 4,500 patients who did not previously have a doctor. Seven new family doctors have been recruited to practice in the area, and local residents who are looking for a family doctor can now be matched to one without any wait time. The Division works closely with Fraser Health, the Peach Arch Hospital and Community Health Foundation and other partners.

Here are some of the local solutions that have produced results:

  • New Primary Care Clinic - The Division opened the new Primary Care Access Clinic in the fall of 2010. It is located at the Centre for Active Living in White Rock. Among the staff are doctors, nurse practitioners and mental health and addictions workers. The team:
    • looks after patients – without a regular family doctor – who are released from Peace Arch Hospital;
    • helps people get access to services such as home health care or mental health care;
    • helps people find a regular family doctor; and
    • provides long-term care for patients whose needs are difficult to meet in a regular family doctor’s office.
  • Grants for Family Practice Nurses - The Division has given 10 family physician offices grants that supported them in hiring or maintaining RNs and LPNs within their practice.  Having this support can help to enhance care for chronic diseases, and increase capacity within the practice.
  • Care for frail elderly - Care for patients in residential health facilities has improved too. White Rock-South Surrey has a team of 10 local family doctors that provides 24-hour care to nine different local facilities. Also in partnership with Fraser Health, services like home health have been more closely integrated with family doctors’ offices to provide seamless care for patients and their families.
